Coaching Agreement

I am delighted to be working with you and look forward to coaching you toward creating the life you really want to have. Please read through this Agreement and reach out if you have any questions whatsoever! 

This Agreement is entered into by and between Suzanne Trainor ("Coach", “Suzanne”), National Board Certified Health & Wellness Coach of Rooted in Being Health & Wellness Coaching, and __________________ (“Client”). 

Suzanne is trained in behavior change, motivational techniques, positive psychology, mind-body medicine, and Functional Medicine as it pertains to health and wellbeing coaching. Rather than acting as an expert and giving advice, Suzanne creates an environment that gives me (“Client”) permission to take the lead. Coaching is designed to help me connect with my own wisdom. As the Client, I will determine my unique goals through a process of self-discovery and accountability. Suzanne facilitates change at a pace set by me, the Client. 

Suzanne will challenge me, ask difficult questions, and interrupt me when needed. She will encourage, support, and believe in me, even when I may not! When appropriate, she’ll provide me with exercises and inquiries to do between sessions. I understand that ups & downs are normal in coaching. The down cycle is where we do the most growth. 

I am fully responsible for my well-being during coaching, including my choices and decisions. I am aware that I can choose to discontinue coaching at any time. Coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education, and recreation. Deciding how to handle these issues and implement my choices is exclusively my responsibility.

Suzanne is not a medical professional, nutritionist, dietician, therapist, or counselor; she does not diagnose or treat any medical or mental health condition, nor does she offer medical or personal dietary advice. She may share information from reputable sources. It is my full responsibility to consult with a licensed medical professional prior to making any diet or lifestyle changes.

Fees/Payment. Coaching fees are $95/hour ($570 for a package of six sessions). Sessions must be prepaid via PayPal. If a coaching package is purchased, Client will automatically be provided with a “Gift Certificate” and coupon, to be used when scheduling individual sessions.

Sliding Scale. Client understands that Suzanne offers a sliding scale and this option was discussed during the Discovery Session.

Donations. As of May 2021, Suzanne will donate 100% of coaching fees she receives to one of six preselected charities. Client decides which charity when scheduling sessions. Client must attend scheduled session for the fees to be donated; fees from missed or forfeited sessions will not be donated.

Booking Sessions. Sessions can be booked directly via Suzanne’s scheduling page (link will be provided) or Coach/Client may agree upon a set weekly/bi-weekly time to meet. With the exception of the initial Discovery Sessions which may take place over Zoom, sessions are conducted over the phone. Suzanne will call Client directly.

Session Packages. If a 6-session package is purchased, sessions can take place every week or every other week. All sessions must be used within a 3 month period (from date of first session). Please book the first session within two weeks of Discovery Session. Email coaching is available between sessions; Suzanne will do her best to respond to questions within 24 hours.

Cancellations. Client agrees that it is the Client's responsibility to notify the Coach 24 hours in advance of the scheduled session. If Client provides Coach less than 24 hours notice, Coach will attempt in good faith to reschedule the missed meeting. Refunding or rescheduling repeated late cancellations will be at the discretion of the Coach.

Ethics. As a National Board Certified Health and Wellness Coach, Suzanne acknowledges and agrees to honor her ethical and legal obligations to her coaching clients and sponsors, colleagues, and to the public at large. She pledges to comply with the NBHWC Code of Ethics and to practice these standards with those whom she coaches, teaches, mentors, or supervises. Confidentiality. This coaching relationship, as well as all information (documented or verbal) that Client shares with Coach as part of this relationship, is bound by the principles of confidentiality set forth in the NBC-HWC Code of Ethics. However, Client does understand that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. Coach agrees not to disclose any information pertaining to Client without Client’s written consent. Coach will not disclose Client’s name as a reference without Client’s consent. 

Confidential information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client’s industry; (c) is obtained by the Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Coach without use of or reference to the Client’s confidential information; or (e) the Coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to the Coach and as a result of such disclosure the Coach reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and (g) involves illegal activity. The Client also acknowledges his or her continuing obligation to raise any confidentiality questions or concerns with the Coach in a timely manner.
